RESIDENCY PROGRAM:

Focus: Family Medicine

Program Highlights:

 

This primary care residency is based at the Family Health Centre, Toronto Western Hospital (TWH), University Health Network (UHN).

 

It is a 12-month program that will provide the resident with experience in a variety of ambulatory care settings.  The current healthcare environment in Canada demands that the primary care system adopt an interprofessional model of care, creating a unique opportunity for pharmacists.  The benefits of an expanded pharmacist role in primary care are well documented; however, many pharmacists have limited exposure and experience in an ambulatory care setting.  This program is designed to prepare graduates for successful careers as independent practitioners of Pharmaceutical Care in an ambulatory care setting.   The resident will have the opportunity to provide leadership in enhancing clinical pharmacy services or in the creation of an innovative practice site.  Opportunities to become involved with teaching, research and other scholarly activities will be available.

 

Required Rotations:

 

The majority of the resident’s time will be spent in the Family Health Centre.  Note that most required and elective rotations can be completed longitudinally over a period of time.  There is some flexibility in the amount of time allocated to each rotation:

  • Family Health Centre (total of about 7 months)
  • Drug Information (4 weeks)
  • Family Medicine Inpatient Service, TWH (4 weeks)
  • Outpatient pharmacy at TWH (4 weeks)
  • External Family Health Team (4 weeks)
  • Research (up to 8 weeks)
  • Clinical initiative - The resident will have the opportunity to provide leadership for an initiative related to enhancing clinical services in a particular area, or, in the creation of an innovative clinical practice site within or outside of UHN.

 

Elective Rotations:

 

The resident has a choice of 2-3 elective rotations.  Elective rotations are 4 weeks in length and may be completed longitudinally. The resident may choose from a number of elective ambulatory care settings within UHN or other institutions.  Alternatively, the resident may initiate a request to complete an elective that is not listed.  Potential electives include:

External to UHN:

-         Community Health Centres (CHCs)

-         Community drug stores

-         Home care/CCAC

-         Specialty clinics (e.g. pediatric ambulatory care, osteoporosis/women’s health)

Available at UHN:

-         HIV clinic

-         Nephrology/dialysis clinic

-         Emergency medicine

-         Cardiac clinics

-         Oncology clinics

 

Minimum required credentials: B.Sc.Phm.

 

 

ORGANIZATION

Type and Sites:  The University Health Network is a fully affiliated teaching hospital consisting of 3 sites: Toronto Western Hospital (TWH), Toronto General Hospital (TGH) and Princess Margaret Hospital (PMH). 

The Family Health Centre is based at TWH and provides primary care to families and community members in the surrounding area, with special attention to the needs of the aging population, children and women.  The centre is comprised of an interprofessional team including physicians, pharmacists, nurses, dieticians, social work and chiropody.  The centre is an academic training site for medical residents and other health professional students.

Number of beds: 744; The Family Health Centre services a population of approximately 12,000 patients.
